This depends whether: The cyst is large enough to compress the spinal cord or not. If it is small and no compression or edema is noted, then will most likely be okay. If it is big enough and causes any compression, then it may need to be removed. If it is causing compression, will usually see some neuro signs like pain, sensory changes or weakness at and below levels, even paralysis if large without surgical intervent.
