Common sense: If it's persistent and non-pleasurable, and the head and part along the bottom aren't hard like the rest, it's priapism and a medical emergency -- get seen at once. If you've just gotten all excited and haven't climaxed after four yours, see if you can do something about the latter, and only get seen if the situation is intractable and/or clearly not normal.
