Do i have to list all health problems on insurance forms?

You should.: Currently in the private market, your insurance premium is based on your health status or any preexisting conditions. If you don't list a preexisting condition, then rack up a bunch of medical bills for that condition, the insurance company (who can request past medical records) can refuse to pay and leave you on the hook for payment. It's best to be up front about your health conditions.
