Pain: Emergent surgery is needed to explore testis through scrotum and fix the affected testis if salvageable as well as the opposite one; the time window for saving torsed testis is about 6-8 hours, so go in a hurry. If the torsed testis cannot be saved, remove it (orchiectomy). Postop pain and swelling are expected and usually resolved nicely after the worst pain over the first/second postop days.
