Occasionally: When the benefits justify the increased toxicity of both given together. Some examples include stage 3 non small cell lung cancer, limited stage small cell lung cancer, locally advanced head and neck cancer, stage 3 rectal cancer, and glioblastoma (brain cancer). There are other situations when the treating doc may feel that the benefit outweights toxicity and do the combo.
