How accurate is conception dates based on an u/s of 11 weeks? what percentage?

Not a percentage: First trimester us is usually considered to be accurate within a week, or +/- 3-4 days. There is no percentage implied, it doesn't work like that. In my experience 1st trimester us can be accurate to within +/-1 day under many circumstances, but you shouldn't count on it every time.
