Probably fat loss: If there is not an ongoing illness, and if a person was doing the same amount of daily activities (even if it is "no" activity) earlier compared with now, then his body is unlikely to lose muscle. If he were more active earlier, and then became bedridden due to illness, then some muscle would be loss.
