Sometimes: There is some evidence that treatment of early appendicitis without a fecolith can be treated with antibiotics alone. However, failure of treatment could result in higher risks of infection and complications. In my mind, its an organ you don't need and the surgery is relatively simple to prevent serious complications, so surgery is still the answer.
