Your neurologist: should have a nurse or other staff member who can direct you to public or private services & funding, depending on the degree of your functional impairment. Ask a family member to visit the doctor with you to dicuss prognosis & expected needs. You may qualify for SSI, Medicaid, case management by a medical social worker,in-home care, etc. If not offered, ask for resources now.
