Just monitor: The nice thing about the mouth and stomach is they kill a lot of nasty things that may try to get in to your system. It is very unlikely that just sucking on a rusty nail is going to cause any problems or illness. Just monitor closely for a few days and if develops any mouth sores, fever or vomiting a visit to doctor is warranted then.

If there was any cut or abrasion as a result of the nail, please see your Pediatrician. Keep immunizations up to date. In the meantime, watch for signs of infection, including fever. Keep all caustic agents in a locked area and keep the Poison Control number available.
