Heard that sometimes nct can come back normal, but there is still a nerve being pinched how can a neurosurgeon determined where the pain is comin from?

Exam and Imaging: A normal nct simply indicates that there is not damage - the nerve can still be pinched. A combination of history, physical examination and appropriate imaging (mri/ ct) should help pinpoint the source of the pain.

Neck CT: The neck (n) ct scan that you refer to is best for defining the boney structures of the neck. This is not likely to show the area of nerve pinching in an 18 year old where it could be a herniated disc. This is best seen by MRI scan. The neurosurgeon can also perform a history and physical and from that determine where the pain is coming from-ie. Where is the pinched nerve?
