Not necessarily: Seeing and hearing things that aren't there, can be hallucinations. Misinterpreting things that are there, can be illusions. Thinking either of these will hurt you can be frightening. Seeing a psychiatrist for a full evaluation would be helpful in figuring out what is causing you to have these experiences. They do not automatically mean schizophrenia -- other causes are possible too.

Speak w psychiatrist: You can't just make the diagnosis of schizophrenia based on this description especially in a 16 year old . However these are significant and bothersome problems which should be discussed with a psychiatrist.
