Blood pressure: Elevated blood pressure and rare vascular abnormalities can cause this. I have also seen it in "normal patients".

Eye specialist: With strong pulsations in the eyeballs and black shadow around your vision, i feel a blood pressure check and an exam by an opthalmologist are advisable.
