Rule out a DVT: The first thing to do is rule out a blood clot. Long periods of inactivity and decreased ambulation can lead to a dvt, which, more importantly can lead to a pulmonary embolism which can be fatal. After that is ruled out, the back pain can be from either tight muscles in the legs and back or irritation of a nerve from mal-alignment of vertebrae after long term poor positioning. Rule out DVT first.
