Can't say: My first impression is this is all muscular, myofascial pain could explain this. You need am evaluation to clarify this.

Neck and Back Issues: Actually your arm pain may be due to something happening in your neck and the hip pain may be related to back issues. Other possibilities can be bursitis around the hip/thigh especially since you can't lay on that side. Since its bothering you i would consider seeing a physical medicine ; rehabilitation specialist or sports medicine doctor to further evaluate you.
