10 mo son with aspiration; we use simply thick. trying slow transition to thins. now has increased coughing and spit-up between feedings (but not during feeding). is he coughing up aspirated liquid?

Aspiration: If you haven't done so already try increasing the number of feeding with smaller amounts at each feeding. Also have the child in a sitting position for at least 1/2 hour after eating. Continue ranitidine after eating.

Question not clear: We do not understand your question.You have 10 month old son with what kind of aspiration an what is thick and thin. If you mean he has acid reflux with aspiration.Baby may need meds for acid suppression and feeding propped up in sitting position and cleeping with head elevated consult a pediatric gastroenterologist if there is history of aspiration continue ranitidine and discuss use of prevacid (lansoprazole)?

GERD: Has he had persistent or recurrent chest congestion? Or recurrent wheezing? Has developed pneumonia or anemia? Poor weight gain? If yes to any of above question, he is a candidate for fundoplication procedure with or without pyloroplasty.
