Decongestant: If the decongestant is not helping and it has been more than a couple of days you will need to see a doctor, most likely your symptoms are self limited and should resolve on their own.

Yes see MD: Any change in hearing should be evaluated to help reduce the chance of permanent damage. See your physician or otolaryngologist (ear, nose ; throat doctor).
