41+3 weeks, 5cm dilated with no contractions. lots of bad cramps and back pain and shaking from pain. midwife won't intervene to help move thins along?

See MD: Most doctors have adopted a cut off of 41+0 for initiation of spontaneous labor, only because the curve (likelihood) for intrauterine fetal demise goes up significantly after 41w, and if no labor by 41w, labor less likely. You may need to be admitted, rupture membranes or start pitocin (oxytocin). Be smart and safe. More important than 'natural' birth is safe, healthy baby and mom.
