It May...: If the antibiotic is not the right one for the type of infection, the problem will continue. If the bacteria that causes the infection is resistant to doxycycline, the problem will remain. You need to wait at least 10 days to have another blood test to see if the infection stopped . If still there , you need to have a test to determine which bacteria(e) is there and which antibiotic will kill it

Yes: It is possible to have an infection while on antibiotics, even possible for that infection to be resistant and untreated by that antibiotics. Cultures may grow more slowly in presence of antibiotics. The blood test may show an indication like an elevated WBC but if the infection is in the blood that would be trouble.
