Yes: If you define rape as the forced sexual contact of a perpetrator upon a victim, this can occur to victims at any age. In the case of a 4 year old this may involve a variety of sexual contacts that include orogenital, genital to genital and anal contact.

Absolutely: A four year old does not have the intelligence or understanding or physical strength to deal with a potential rapist, but does have a vagina which can be used for this purpose. Unfortunately a lot of these rapes occur within family units with predatory family members involved. There are specialists in children's hospitals in large cities who are trained to deal with this possibility.
