Many reasons: Unfortunately I have missed a few deliveries in my 15 years as well. Traffic, trains, nurses waiting too late to call, women who gave birth easily and rapidly, and on and on. Don't be to hard on your doc - this happens. Raz him a bit about it but i bet he's frustrated he didn't make it either - we like delivering! what matters is your child was hopefully delivered safely - enjoy the bonding time!
