Why would thyroid go from massively underactive to massively over active in three weeks?

Depends : If an underactive thyroid was treated with medication and the dose was higher than needed the followup labs would look like the thyroid was overactive but dose just needs to be decreased. Autoimmune thyroid disease can also cause rapid shifts or make it difficult to regulate medications. To determine that thyroid antibody testing needs to be done. If positive tx in addition to meds can help.
