See urologist: The hernia should be addressed by a surgeon, but only after the cause is determined, e.g. Prostate enlargement, straining to void, etc. He can fix your hydrocoele at the same time. Likely your age and diabetes has contributed to the ed, you need a physical to make sure there are no other reasons, like blocked arteries to the penis, but there are medications, oral and injectable for this.

Hernia/Hcele surgery: Will take care of hernia ; hydrocele. Erectile dysfunction may respond to medication, vacuum device or best be served by surgical implantation of a penile prosthesis. Best to consult with a urologist who may be able to manage all three problems. Otherwise may be referred to a general surgeon for hernia repair.
