Unknown: Despite multiple attempts at diagnosis, there is no sure diagnosis of the disease joseph merrick suffered from. It was thought for many years he had nf, though he may have had proteus disease. There are numerous conditions that can cause extreme deformities that look similar.

Most likely: The features of both are similar in some respects but quite different in others. Since the elephant man has long passed, his DNA is not available to probe for definite analysis. Having cared for patients with the Proteus syndrome who shared multiple features of the EM, I think it leans more in that direction.
