Soft tissue: Chronic neck ; posterior shoulder pain in thirties male suggests muscle spasm and tendonitis. In absence of acute mechanical injury, repetitive job stress needs consideration. Best doctor to evaluate an osteopath. Imaging studies might be in order, especially chest x- ray to rule out lung tumor. Chronic abdominal symptoms ; use of Bentyl (dicyclomine) suggest ibs, which is associated with fibromyalgia.

disc problem: Depends on duration, but chronic pain in that pattern could be caused by a pinched nerve in the neck. A detailed physical and neuro exam, if neck movement or talking on the phone in that arm aggravates symptoms, it is consistent, an MRI might be useful, if it is affecting performance or function. Otherwise anti inflammatories, rest about the neck with cardio low impact exercise useful.
