How can you miss triplet on ultrasound?

Depends: Ultrasound is not a perfect technique and sometimes it is not surprising to miss a triplet pregnancy, most o the ultrasound people are highly trained and certified, yet it is hard to prepare for absolut everything that nature can present to you. So be mindfull and understand that it is within our nature and technology to miss things sometimes.
