What should be included in a personal "health check list" at the start of the year and why?

Basic info: Vital signs" weight, BP , pulse and height. Current meds, drug and food allergies. Past hospitalizations and surgeries. History of family illnesses, and social situation including exercise, drug and alcohol use. Include amount and type of regular exercise as well as diet.
