Not usually: Most endometriosis can be diagnosed only be visual inspection, perhaps with a biopsy to confirm it. Endometriomas which are endometriosis cysts can be seen on ultrasound but usually indicate more advanced disease.

No: No. Endometriosis is diagnosed based on symptoms or during surgery. The only way to for sure know it is endometriosis is to see it at surgery and biopsy the spot.
