Ectopics: An ectopic pregnancy hurts as it gets larger. A miscarriage hurts as the uterus cramps. This is one of emergency medicines dilemmas, to tell the difference between the two. You need to see your gyn if you have this problem in early pregnancy.

Not always: An unruptured ectopic pregnancy in its' earliiest stages may not have any symptoms at all. Some "miscarriages" in its early stages are similarly asymptomatic.
