No way to know: There is no way to judge the severity of bipolar disorder by the dose prescribed to the patient. The dose that you give however is only 4 mg and lithium is generally prescribed in doses of 300 mg and above.

Lithium orotate?: This dose of lithium is extremely low, -- probably not enough to effectively treat bipolar disorder. Maintenance doses of Lithium Carbonate and citrate are generally in the range of 400-1200 mg/day, with blood levels in the range of .6 - 1.0 mmoles/l.
