Changes in hygiene: Several possibilities. First, the person could be depressed -- showing little interest in, or energy to attend to, grooming. Another possibility is psychosis, because the person may be hearing voices telling him or her not to take baths -- or attention is distracted elsewhere. A third possibility is dementia. A medical evaluation can help discern what's happening, and treat as needed.
