Does a heart attack always have noticeable symptoms

Not always: A "silent heart attack" is one that occurs without any symptoms that are recognized by the patient who is having it.

No: Many times people have "silent" heart attacks which are detected later from other problems or repeat non-silent heart attacks. Any change in condition, especially suddenly, should be evaluated and especially if you have risk factors.
