See a foot doc: There is a good chance he broke the toe. The purple color should go away over time. Ice the toe if the injury is within 24 -48 hours.....X-rays should be taken.

Depends: If it is only a small blood blister with no signs of infection then soak it in warm water to soften the skin and see if it will drain on its own. If not see a podiatrist for care.
