Muscle tension : Many of your symptoms sound like muscle tension dysphonia. Too much muscle tension in the throat can cause all those symptoms as well as lump on the throat and hoarseness. An ENT can look in the throat with a scope to make sure all is well. You may need a speech therapist to help.
