You can't- see Doc!: It can be either, with the information given. Can it be an infection or cancer? Don't rely on our answers in this forum- further description is necessary to accurately answer your question. Because of fever, i would recommend that you see your doctor or go to an urgent care center to answer your question since it may require further questioning and studies to diagnose and treat.

The following:: Kidney:pain radiates round or down to groin. Tenderness to tapping or thumping over lower rib cage on back. Uti symptoms of burning with or frequency of urination. Blood in urine. Positive urinalysis. Kidney or bladder abnormalities on ultrasound or ct. Elevated BUN or creatinine levels in blood. Intestinal: change in stool, bowel habit & pain with bm. Bloody or tarry stool. Abnormal bowel x-rays.
