Mid-late adolesence: This condition is seen in active children and manifests as pain with abnormal bone formation at site of insertion of patellar tendon. May be related to increased stress applied to growth plate in this area. Once growth plate closes as person matures the process ceases. However a prominence will always remain and if a seperate bony ossicle is present there may be persistent pain into adulthood.
