see details: Is the pain due to damage or continued inflammation? If it is secondary to damage i would discussed options with your rheumatologist and at least consider working with a hand therapist. If it is due to chronic inflammation, you need to consider the addition of a tnf agent. Tobacco use makes RA worse so you should stop that habit now.
