pain: If it is an infected tooth it will get worse and can have more long range effects ( heart, muscles etc ) off and on most likely means it is in earlier stages. Go to the dentist asap.

Problems: An infected tooth only gets worse over time, regarding pain, swelling, and discomfort. The sooner you see a dentist the sooner you will feel better.
