Menopause: As woman get closer to menopause periods often become irregular. She may skip some periods or have a bleeding pattern different than what she is used to. Fertility drops as women get older so chances of pregnancy are very low at this age, but not impossible.

Not likely: In US, average age for menopause is 51. If she has not started menopause, she could have irregularity as it approaches. It is not likely, but pregnancy test would answer the question for sure.

Very unlikely: Could just be the end of it (menopause), at any rate home pregnancy test can be done to be sure she isn't, best wishes
