Your choice: As far as do or md, that is a choice that will be made by you depending on the personality and "dr-pt relationship" you develop with your dr./gp. With chronic neurological issues, you may be better served seeing an internist as your primary care provider, however i know many board certified family medicine docs who are also very capable. Good luck.

MD or DO: Internists (internal medicine doctors) are some of the best primary care doctors for complicated medical illnesses. I am partial of course. Do or md is usually not a huge distinction. Find out where they did their residency (was it an acgme certified internal medicine program) and get personal recommendations.

Doc choice: I am an md internist and have a natural presence to people trained as i was. Many however would say there is no significant difference between mds and dos. I would base my choice on where the person did his residency training, and would personally choose the physician trained in an academic center.

All good: Doesn't matter DO or MD, both are highly trained and can help you if they are primary care physicians. same with family and internal medicine.
