Renal insufficiency: The BUN and creatinine should not be elevated due to the pneumonia unless the pneumonia is non-bacterial (autoimmune), or due to group a strep pyogenes strains that induce glomerulnephritis. Generally the BUN and cr will be elevated in febrile patients with infection when they are dehydrated and corrects rapidly with fluid replacement. Also in septic shock there may be acute tubular necrosis.
