See your PCP: A referral to an ENT is in order. Holes in the septum can come from many causes: infection, chronic rhinitis, decongestant nasal spray abuse/overdose, cancer, chronic nose-picking, etc. In early stages, a graft procedure can be performed using tendon from another site in your body. Once too large, it can only get worse--eventually can lead to collapse of the bridge of the nose. Treat it early!
