Advanced imaging: If you have not had advanced imaging recently (mri or ct myelogram), and you have already had interventional pain management (injections) and physical therapy, you should update your imaging and see a surgeon to discuss treatment options. Long term narcotic (hydrocodone) use is not the recommended treatment for spinal stenosis.

Spinal cord stimulat: If surgical options are not viable or unacceptable, then spinal cord stimulator can help. If that also fails then long acting opiod medications added to short acting is also an option. Finally morphine pumps in spinal canal, like Insulin pumps, are often successful in controlling intractable pain and keeping the person functional in adl.

Steroid pack: You ought to talk with your pcp about talking a steroid dose pack. If it doesnt get better, you should get an MRI and follow with a spine surgeon if indicated.

Nothing: Leg pain cannot be treated with the connex or anti-inflammatory. Most likely you would require an evaluation by an orthopedic spine surgeon and be considered either for epidural steroid injections or a lumbar microdiscectomy procedure to take the pressure off of the nerve.

Spinal stenosis: If you are experiencing increased radicular pain to the bilateral lower extremities due to lumbar spinal stenosis, you should discuss this issue with your local neurosurgeon or orthopedic spine specialist. You should also consider other treatment modalities such as physical therapies, spinal decompressive therapies, or lumbar epidural injections. Surgery will be reserved as a last treatment option.
