No one can tell: A smelly urine is probably due to infection, but not necessarily. If there is infection however, the only reliable way to sort the type of infection out is to do urine culture. The so called empiric treatment is usually tried in simple cases, without culture, targeting a group of possible bacterial organisms depending on the scenario e.g. Older vs younger sexually active persons.
