How do you check if someone really passed out?

Basic CPR: Check to see if they're breathing and that their airway is unobstructed. Check for a carotid pulse. If either is absent, initiate cpr. If present and they don't respond to voice, it is customary to apply a mildly painful stimulus - like a gentle pinch. If you suspect malingering, "gentle" may be subject to interpretation.
