Not necessarily: If your doctor is changing your medication, it may be because it wasn't serving your needs?  Usually changes are made when people's symptoms don't respond, and better results are needed.  I wouldn't expect there to be a "down cycle" unless Lexapro (escitalopram) was helping you more than you realized, and zoloft not so much -- or higher dose is needed.  Please contact your prescribing doctor about this.

Their are many different causes of incontinence and it can usually be treated if it is a problem for you.  The treatment would depend on the underlying cause of your leakage. no: I don't know if you mean once, by mistake, or regularly.  If you doubled your dose by mistake once, it's unlikely to be harmful, although you might suffer serotonin side-effects (stomach upset, jitteriness, restlessness).  Taking 300 on a regular basis usually does more harm than good.  It's not okay if your doctor didn't prescribe it that way.

Difficult.: Patients are usually on these medications for anxiety/ depression symptoms.  It does alter the brain chemistry, so to allow the body to slowly adapt to not having it is tricky and can take months to years.  You could feel a rebound of the original symptoms and/or other effects if you stop too quick.

Multiple causes: Bladder infection (uti), constipation. Sometimes hereditary overactive bladder. Extreme physical stress in some athletes esp if high impact sports such as track, gymnastics & basketball. Pelvic muscle dysfunction, can be weak or dysfunctional hypertonic (spastic) muscles leading to loss of bladder control.

No: Not at all, some types of incontinence occur prior to puberty. Stress incontinence becomes more common as women age, but can happen much earlier.
