for centuries children and adults did ok treating puncture wound of the finger with home remedies. why can't i?

Old ways best?: You might do just fine treating yourself but sometimes people don't which is why we now have doctors.  Centuries ago many people died of preventable things such as infections from puncture wounds or tetanus.  I'm not saying every wound has to be seen by a doctor but if you have serious medical issues or it is complicated (i.e. Dirty, numb or excessively bleeding) or tetanus prone, then see md!

one of the children at the orphanage i care for has a healing wound from an incision. it's not healing how i would like, and i think we need to switch from the current topical ointment (mupirocin). i'm looking for recommendations?

: Surgical wounds that are not healing should be taken to the attention of the operating surgeon.  The location and reason for the surgery are important to know before suggesting anything more than soap and water.

Yes: Although fertility decreases as you get older, the effect is not particularly noticeable until your 40's.  If you have been trying without success then you need to be checked.

Sore bottom: Small tears are very common, especially with a first baby.  Most women require a few stitches.  Larger tears may require stitches in the muscles of the pelvic floor or around the anus.  These repairs heal very quickly and by the 6 week postpartum visit they typically look good as new.  You may need stool softeners if you have a large tear and expect a sore bottom for a few days to a few weeks.

Usually, no.: Only safe, "generic" answer is that you should decide this in conference with your gyn dr after an assessment. (and your partner, of course.) as women pursue advanced degrees and career standing, pregnancy is routinely falling into the 30's and sometimes 40's. It's what is safe / healthy for you.

Depends where: The bullet entered, the caliber and proximity the injured person was to the weapon.  A bullet wound to the head is treated differently than one to the neck, chest, abdomen, or extremity.

STD's and PID: Sexually transmitted diseases can lead to pelvic inflammatory disease.  PID can lead to later infertility.
