Breakthrough bleed: is possible; however, if having severe bleeding get checked and if you miss a complete cycle- get a pelvic exam. Worth rechecking pregnacy blood test if no cycle next month.

: With documented PCOS, it is unlikely for pregnancy to occur with the information described.  Taking Primolut with expected post treatment bleeding is normal, and generally rules out pregnancy.  Obtain a serum HCG test to be certain about pregnancy, and ask your gyn about low dose oral bcps, which is the appropriate treatment for PCOS until pregnancy may be desired.

Very unlikely: The period after plan B is usually different from your usual periods and with the proper use of planB and 3 negative pregnancy tests it is highly unlikely that you are pregnant. You do need to get in and get started on a reliable method of contraception so you don't have to go through this worry again. Be safe.

Painful sex: There can be multiple causes. Dryness is one of them. More time to foreplay increases lubrication of the vagina. You can also use water based lubricant. Also decreased estrogen in the blood increase dryness of the vagina. You can get that checked. If it is low estrogen hormone supplement can help. Another cause is vaginismus which to treat is much more difficult. You need to see an expert gyn.

: With repeated vaginal bleeding and negative urine pregnancy tests then pregnancy is most likely ruled out. Frequent urination is not a symptom of early pregnancy and this symptom is quite common. If UTI is ruled out then you should control frequency with cognitive behavior therapy (CBT). You can google CBT for overactive bladder (OAB). Good luck.

: If what is meant by "cryptic" is the word "ectopic" pregnancy, the bleeding from ectopic pregnancy is caused by irregular hormone production by poor blood circulation by implantation at sites outside the uterus.  An ectopic pregnancy produces often low levels of pregnancy hormone, so a serum HCG test is the most reliable to determine a positive  for early ectopic pregnancy.

Take a test: If you think you might be pregnant check then check a home pregnancy test. They are cheap, reliable and available  at any pharmacy. Some irregular bleeding happens sometimes after emergency contraception. Starting birth control would help control your bleeding patterns.

: No, it is not necessary to have withdrawal bleeding, but that is a very incomplete answer without more information such has last menses and current and recent medications. It does not suggest pregnancy but that cannot be ruled out either.
