Thumb injury: Go to the emergency room to have it checked. An xray can check to see if there is a fracture. They can puncture the top of the nail and relieve the pressure from the blood under the nail.

Could be, yes: Yes it could be broken.  Obviously the first priority is to stop the bleeding.  Direct pressure should help.  If you are concerned about a fracture, an x ray will be needed.

When there's pus: Depending on what was on the fork, and the fact that it's metal, means making  sure that you've had a tetanus booster. Also, with the puncture you are going to experience pain and throbbing. So if there's any signs of swelling or inflammation, or the skin is red, it needs medical attention. If it's a deep puncture, and you irrigated and applied soap, you still may need to see a doctor to start oral antibiotics.

The treatment: Depends on the diagnosis. Is it dried blood? Fungus? Bacteria? Melanoma? If you don't know then you need to see someone that does. Dried blood is no big deal but something like a melanoma is....

Soccer technique key: Soccer often puts a lot of trauma and stress on toe  nails.
You can decrease the chances of this by keeping your toe nails trimmed and making sure your soccer shoes aren't too tight.

If your on a soccer team, work with your coach to improve your technique. You should be kicking with the inner side of your foot, not your toes. Better technique should help prevent this as well. Visit your PCP.

Ice, drainage: Usually it can be treated with ice to decrease the swelling
along with nsaids
but if swelling is more painful , a small hole can be placed
in the nailplate to decrease the swelling and therefore help
the pain.
