A : A rash that has been treated with all these therapies is very difficult to make suggestions for further therapies without seeing the rash. I would have expected a typical diaper dermatitis due to yeast to have cleared with the ketoconazole. I would be hesitant to use a stronger steroid under a diaper. I would suggest making an appointment with your pediatrician or a dermatologist. Emergency room doctors are generally not use to treating diaper rashes. Rashes just have to be seen to diagnose accurately and treat appropriately.

See a dermatologist: You have a wrong diagnosis. Forget about yeast. You probably need a cortisone solution such as fluocinonide solution .05% in a 60ml bottle. See a skin specialist for a diagnosis.

Should be OK: Hopefully the yeast infection has improved on ketoconazole, and switching to Miconazole should be fine. They are quite similar in their antifungal properties. If you do notice, however, that the rash starts to worsen on miconazole, i would get a new prescription of the ketoconazole.

Ketoconazole: Ketoconazole cream is an antifungal agent that works against yeast.  Clobetasol is a strong steroid that will supress inflammation and make it seem better but it will not kill the yeast, just hide it.  Also clobetasol is extremely strong for the penis. It will cause skin atropy.
